ABOUT ADDISON COUNTY HOME HEALTH
& HOSPICE
Addison County Home Health and Hospice,
Inc. (ACHHH, Home Health) was founded in 1968 by a committed group of
Addison county residents to fill an unmet need for community based
professional home care services. It was the intent to make a broad
spectrum of home care services available to families and individuals of
all ages, regardless of extent of illness or injury.
Governed by a volunteer board of directors, Addison County Home Health
and Hospice, Inc. is a non-profit agency certified by
Medicare to provide comprehensive home care services in Addison County,
Vermont.
The professional services of Addison County Home Health and Hospice,
Inc. are skilled nursing, physical therapy, speech therapy,
occupational therapy, medical social work, and nutritional
consultation. Ancillary support services include home health aide,
personal
care attendant, and homemaker.
Addison County Home Health and Hospice office hours are 8:00 AM to 4:30 PM. Home visits may be made to patients
during a more expanded period of time.
Evening hours, weekends, and holidays
are covered by a team of nurses who are available for scheduled visits
and to respond to specific
patient problems.
Referrals for home health care are
received from physicians, hospitals, and nursing homes.
If a patient or family member feels that an individual needs to be
referred for care, they should either call their primary physician
or call Home Health and ask for assistance.
