About Us
Meeting a Need
In 1968, a group of Addison county residents founded Addison County Home Health and Hospice, Inc. (ACHHH, Home Health) to fill an unmet need for community based professional home care services. Their intent was to make a broad spectrum of home care services available to families and individuals of all ages, regardless of the extent of illness or injury.
Governance
Addison County Home Health and Hospice, Inc., is a 501(c)3 nonprofit agency governed by a volunteer board of directors and certified by Medicare to provide comprehensive home care services in Addison County, Vermont.
Hours
Skilled services are available to our patients 24 hours a day, 7 days a week.
Our office hours are 8:00 AM to 4:30 PM Monday through Friday, but we have a nurse on call 24 hours a day after normal business hours, on weekends and holidays.
