Community Outreach
For over thirty years, Addison County Home Health and Hospice has been providing clinics at the senior meal sites operated by the Champlain Valley Area Agency on Aging. We offer blood pressure screenings, patient education and maintenance foot care. These services are provided free of charge and are directed toward the participants at the meal sites. Members of the public over the age of 60 are welcome to attend the clinics until they can establish an alternate source of foot care and nail care. Please visit our Foot Care and Blood Pressure Clinic Schedule for dates and times.
In addition, we have been administering influenza vaccines in clinics, which are open to all adults in Addison County, in partnership with the Vermont Department of Health and The Vermont Office on Aging. Please visit our Calendar of Events page for Flu Clinic dates and times.

Although a sorrowful time, my father's passing was marked with dignity and respect to the end” —Joanne
