RN Case Manager- Long-Term Care: The position provides an exciting opportunity to the right RN to join

us providing assistance to our elderly and adult disabled population. You will assist them in receiving services

in their home setting by initiating, organizing and monitoring all of their supportive services. This is a 20-30

hour position.

You must hold a valid VT nursing license and have a minimum of two years of medical surgical experience for

this nursing position. The agency offers you a quality of life in both personal and professional endeavors.

For Community Health RNs there is no required weekend or evening work shifts. We additionally provide

you with a quality benefit program if you are working at least 30 hours/week and a very generous time off

program.
