Privacy Notice-- Addison County Home Health and Hospice, Inc. is required by law to maintain the privacy of all patient health information and to provide the patient (or his representative) with a copy of Agency privacy practices. The federal law, the Health Insurance Portability and Accountability Act (HIPAA) of 1996 states that Addison County Home Health and Hospice, Inc. may use patient health information for the purposes of providing the patient with care, obtaining payment for the care and conducting agency health care operations. Established agency policies guard against any unnecessary disclosure of health information. On admission to agency services, patients are given specific information regarding our privacy practices, as well as information on patient rights and responsibilities.
