​Addison County Home Health and Hospice has recently acquired a 2,963 square foot five-bedroom single-family home located at 427 E. Main Street in East Middlebury. We have been granted a Change in Use permit to transform it into a residential care home for up to eight medically frail adults, and are hoping to raise $500,000 to complete renovations designed for safety, compliance with state regulations and comfort.
Our residents will live in a home-like setting, with 24-hour staffing, where they will receive nursing oversight, three nutritious meals each day and snacks, medication management, assistance with activities of daily living (ADLs), laundry services and opportunities for socialization and engagement. Care will be provided in accordance with state guidelines around safety and well-being, with a person-centered focus which honors residents’ preferences.
We will be providing care coordination for those who may not have other options for supportive living arrangements. An example would be a person who remains in a hospital or nursing home, which provides a higher level of care than they need, because of a lack of available options after discharge. We will create a cohesive team for each resident including nursing, therapy and social work staff, and community partners. Our residents will have a variety of payor sources, including private pay and Medicaid.
The home is located on a main road and has a beautiful porch on two sides, perfect for enjoying fine weather and watching the world go by. There are sidewalks on the street, the land is flat and safe for walking and there is a small park just around the corner. It is within easy driving distance of physicians' offices and Porter Medical Center. The interior features plenty of room for socialization, greeting visitors, and enjoying privacy when desired. Thoughtful renovations will ensure that the structure is accessible and safe for those who reside there. We are working with Salamander Construction, a well-regarded local builder with extensive renovation experience, to make the building eligible to be credentialed as a Level Three care home.
Addison County Home Health and Hospice has been providing home care and hospice care for Addison County residents in their homes since 1968. Our nurses, social workers and therapists become trusted partners in patients' and families' lives and they are often involved in the difficult conversations that can occur when a loved one is no longer able to live safely in his or her home. In Vermont, we are experiencing the same lack of housing which challenges our entire country.
Addison County has one skilled nursing facility and it has a long waiting list. We also have many residents who need help with activities of daily living but do not require the intense care provided in that setting. There are limited options for placements for medically frail individuals in our community, and we have had numerous instances when Addison County residents have had to move out of the county and occasionally the state. We feel strongly that our neighbors deserve to remain in their beloved community.
We have spent the last year discussing this project with community members, leaders of our local institutions and representatives of our state government. All are in agreement that this is a much-needed project. In community meetings discussing housing, the need for housing for medically frail community members is always raised. We are very excited about making a real difference in our community. We recognize that we cannot fix the housing crisis, but we are committed to doing our part to help the most vulnerable segment of our population. Please consider joining us in this work.
