As a Home Care & Hospice Nurse at Addison County Home Health & Hospice, you will play a vital role in delivering compassionate and skilled nursing care to patients in their homes. This position is essential in enhancing the quality of life for our clients while adhering to established standards of practice.
Responsibilities
- Provide direct nursing care to patients in their homes, ensuring comfort and safety.
- Conduct thorough assessments and develop individualized care plans based on patient needs.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and adjust care plans as necessary to optimize outcomes.
- Educate patients and families about health management and self-care techniques.
- Maintain accurate medical documentation in compliance with regulatory standards.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
- Provide end of care services for patients and their families including pain management.
- Valid nursing license in the state of Vermont.
- Experience in home health or hospice care preferred (but not mandatory).
- Strong skills in pain management and acute care nursing.
- Excellent communication skills for effective patient interaction and education.
- Ability to work independently while managing multiple patient needs effectively.
- Time management to ensure all documentation is complete timely.
