Intake & Triage Nurse


Job Description Summary

Collaborates with health care team members, patients, and families to ensure a smooth transition back to the home environment from inpatient facilities. Assesses client needs / issues over the phone. Uses clinical decision making to triage health issues and informs client of appropriate intervention. Assists client to navigate the health care system.

Essential Job Functions/Responsibilities

1. Assesses community and referral source needs to ensure care needs are met.
2. Assesses client needs over the phone and triages orders.
3. Collaborates with health care team members, patients, and families to ensure a smooth transition to the home environment from inpatient facilities.
4. Develops professional and credible relationships with discharge planner, social workers, physicians and associated healthcare workers involved in the referral process.
5. Engage in marketing by educating patients, families, facilities, and physicians about the range of services that ACHHH provides.
6. To be a patient advocate.
7. Explain in detail all options of care to patients and their families to ensure the best possible outcome.
8. Collaborates with all members of the health care team to ensure the patient status, orders, and care needs are adequately addressed.

9. Receives calls from patient, physician, and health care facilities and prioritizes care needs and coordinates care with the team.
10. Addresses any questions or concerns expressed by the patient or family.
11. Promote agency services through telephonic means
12. Educate physicians, discharge planners, social workers and health care staff to ensure equipment and other needs are met before discharge when possible.
13. Evaluate patient and family care needs.
14. Evaluate response to triage instructions and make recommendations as needed,
15. Assess Telehealth Vital Signs. Measurements and communicates with patient to triage symptoms and notifies MD of any vital signs outside the parameters.
16. Fax monthly Telehealth Report to the PCP.
17. Other duties as assigned.

Job Type: Full-time


Pay: $72,000.00 - $77,000.00 per year
