Addison County Home Health & Hospice (ACHH&H) is committed to the protection of our patients from the spread of infectious disease. We have implemented screening protocols for every visit and we are using appropriate Personal Protective Equipment (PPE) when indicated according to the CDC, CMS and state guidelines.
Are you funded by the government? |
No. We are a private, nonprofit organization that provides skilled care to patients and direct bills a variety of insurance plans including Medicare and Medicaid. |
How can we receive care from your organization? |
Ask your physician to refer you for services or call us and ask to speak to a clinical team leader. |
How do I know if I am eligible for care? |
We encourage you to call us for information and to discuss your specific concerns. |

How can we tell when it is time to think about hospice services for our parents, grandparents, family members? |
Please call the agency and ask to speak directly with one our Hospice Director. That discussion, on the phone or in person, will help you and your family understand what decisions need to be made and which of our services may be beneficial. She can also assist you in obtaining a hospice referral. |
How can I figure out what community resources and services are available to me or my family? |
Our home care social worker will speak with you by phone or meet with you in person to discuss what resources and services are available to your family. Please call the agency to find a time that works for you. |
What do I do if I want to change the schedule or frequency of the skilled care visits I am receiving? |
A clinical team leader will assist you in making any adjustment to your current arrangements. Please call the agency to contact our team leaders. |
I need to hire someone to clean my parent's house and stay with him/her at various times. I don't know where to look for someone. Can you recommend someone? |
We may be able to provide this care through our Private Duty Program or our Medicaid Choices for Care Program. Please call us so we can help you personally. |
I live out of state and am concerned about the ongoing welfare of my parents in Addison County. What services might they be eligible to receive? |
We will speak with you extensively about your parents and help you develop a plan that meets their needs and yours. The plan may include referrals, which would allow us to provide care and oversight for your parents in their home, or it could result in us assisting you in obtaining services through other community resources. |
