How does my donation make a difference?
Donations are critical in helping Addison County Home Health & Hospice (ACHHH) fulfill our mission of delivering high quality, comprehensive community health care for patients of all ages regardless of their ability to pay. For more than 50 years we have provided hope, caring, and support to patients and families.
Your donations help to support our existing programs and aid us in our commitment to health care best practices and technology. Many of the services we provide have no funding from outside sources. It is only though your support that we are able to provide:

- Charity Care: Providing high quality care without regard for a person’s ability to pay.
- Community Clinics: Offering more than sixty free foot care clinics in several locations throughout the county.
- Telemonitoring: Allowing patients with chronic disease to monitor their own health at home with daily nursing support via phone in addition to scheduled personal visits.
- Our Commitment to Professional Development: Financial Support helps train staff in specialty services, expanding staff expertise in the most up-to-date healthcare.
- Community Education: Sharing staff expertise in prevention and disease management, with presentations given to residential facilities, civic organizations and other organizations. Additionally, our social work staff also regularly educates community members about advanced care planning and end-of-life issues.
