Community Health Nurse

ACHHH is seeking a Community Health Nurse that will provide skilled nursing services to agency patients based upon a plan of care approved by the physician.  A valid license to practice registered nursing the in the State of Vermont is required.  This is a full time position with an excellent benefits package including medical, dental, vision, life/voluntary life insurance; paid holidays, 401(k) savings plan and paid time off.

This full-time nursing position requires:
 
•Keen patient assessment and critical thinking skills
•Participative, collaborative case management, working with staff and the community
•Commitment to quality patient outcomes
•Supporting evidence-based practice
•The knowledge to prioritize your case load and create an individualized plan of care for the patient
•Working with a diverse population
•Attention to patient needs throughout their health trajectory
•Commitment to life-long learning
•Ability to travel throughout Addison County
•Current VT nursing license
