Hospice/Palliative Care Nurse

Are you the type of nurse who is passionate about patients who need end of life care, hospice care, or chronic symptom management?
If so, then this Nurse Case Manager Role may be the right fit for you! 
Come join a team of hospice and palliative care professionals who practice in an environment that is autonomous, professional, and part of a multidisciplinary team. Our hospice clinicians make a difference in the lives of Addison County residents. The hospice and palliative care patient population consists of not only oncology patients, but many chronically ill patients who need your expertise.

This is a full time position with benefits.

Required minimum education:
  • Associate degree, bachelor’s degree preferred
Required experience:
  • Nursing: 3 years
Required license or certification:
  • Registered Nurse (RN)
