Intake Coordinator / Triage Nurse

We are seeking an intake nurse.  You will work closely with our referring partners in the community, including hospital discharge staff, emergency rooms, and physician practices.  Providing assistance to these partners as necessary, you will collect and assess the medical information necessary for admission to home care.  You will accurately summarize that information for ACHHH clinical staff and identify those patients that need immediate attention for prompt assignment to appropriate ACHHH clinical staff for admission.  You will:

• Receive and verify referral information
• Coordinate with the Sr. Clinical Manager/Clinical Manager and Case Managers regarding patient needs and care
• Validate pertinent information
• Communicate with referral sources all discharge information for CMS regulations
• Follow through with directives from Sr. Clinical Manager/Clinical Manager and Case Managers for completion of information

Excellent communication skills and the ability to work as part of a team are crucial.  Familiarity with the electronic health record and medical coding required, with preference for experience in home care.  Ability to stay focused while constantly being redirected.  Registered Nursing experience required.
