Addison County Home Health and Hospice is looking for a qualified LPN to lead our concierge nursing program. This individual would be responsible for an array of community nursing needs, including foot care, blood draws, private duty nursing and assessments, in home vaccinations and vaccine clinics.

This candidate will also work in the intake office, helping to triage calls, when community needs have been met. The right candidate will have strong customer service and communication skills, work well with others and truly live the ACHHH mission. Our patients always come first and we truly believe that caring for our community is one way we can give back to those who once cared for us. This position will allow for our community to remain in their home while getting the critical care and support they need.
