Medicaid Waiver Case Manager, RN
This position is responsible for the delivery and coordination of long term care services to individual patients who qualify for Medicaid Waiver Services. This person is responsible for assessing the patient’s physical, emotional, and social level of function upon which the patient’s need for Waiver Services is determined. Collaborate with Agency staff to facilitate continuity of care between home care services and long term care waiver services. Efficiently manage patient caseloads by utilizing various resources and ensuring all documentation is captured efficiently.
Work collaboratively with various agencies and personnel to plan Waiver Services; such as, supervise Personal Care Attendants (PCA), Addison County Waiver/TBI team members, Schedulers, and Social Workers. Assure compliance with state, federal and accrediting body regulations and standards.
Represent the Agency professionally, demonstrate the mission of the Agency, provide consistent and comprehensive communication, and utilize decision-making and problem-solving skills. Demonstrate skills and knowledge as required in the Community Health Nursing (Home Health RN) job description.

Qualifications:
- Educational: Successful completion of a nursing program accredited by the National League for Nursing. Baccalaureate degree in nursing preferred.
- Licensure: Holds a license to practice as a registered nurse in the State of Vermont, or hold a temporary license issued by the State Board of Nursing, not to exceed 90 days to permit the practice of nursing by a registered nurse currently licensed in another jurisdiction of the United States and who has applied for licensure by endorsement in accord with 26 V.S.A. Chapter 28, Subchapter 1.
- Experience: Minimum of two years of nursing experience required, home health experience preferred.
