Occupational Therapist
Job Description Summary
Provides occupational therapy services to agency patients based on a plan of treatment which is developed by the therapist and approved by the physician. In the delivery of treatment, the therapist uses independent judgment based on common principles of pathophysiology and accepted standards of care. Works collaboratively with patients, families, other disciplines and other community agencies.
Responsibilities
1.Uses specialized occupational therapy skills to assess the patient’s physical, emotional and social level of function upon which the patient’s need for skilled occupational therapy services and non-skilled services is determined.
2.Initiates diagnostic, preventive and rehabilitative procedures as appropriate to the patient’s care and safety.
3.Makes referrals to other disciplines or other community services as indicated.
6.Efficiently manages a patient caseload.
7.Participates in patient care conferences when appropriate. Maintains communication with other health professionals involved in the patient’s care.
8.Cooperates and consults with appropriate staff to provide staff education when requested and arranged by the clinical director.
9.Treats patients to relieve pain, develop and restore function and achieve maximum performance using appropriate occupational therapy treatments, modalities and procedures as stated in the occupational therapy plan of care.
10.Observes, records and reports the patient’s reaction to treatment and any change in the patient condition to the physician and/or clinical director and other staff involved.
11.Teaches patients, family and other health team personnel including, when appropriate, home health aides, in the phases of occupational therapy in which they may assist the patient; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate.
12.Supervises the home health aide and revises the plan of care as appropriate. Communicates plan of care changes to the aide and scheduler as often as necessary.
13.Participates in the agencies quality improvement program of the agency, as requested.
14.Re-evaluates on a regular basis, the rehabilitation needs of the patient; initiates, develops, implements and makes necessary revisions of the patient’s plan of care. Continually reassesses the patient’s care needs.
15.Keeps abreast of occupational therapy trends and knowledge for service provision, documentation and care coordination. Participates in in-service programs.
16.Participates in state and community initiatives to establish new programs or to evaluate and modify existing programs for families and children, as requested.
17.Other duties as assigned
Position Qualifications
1.Educational: Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association; and is eligible for or has taken and passed the National Registration Examination of the American Occupational Therapy Association.
4. Knowledge and Abilities:
a. Demonstrates knowledge and skills necessary to provide care to and communicate with individuals over the life span.
b. Demonstrates knowledge of the principles of growth and development over the life
span.
6. Must be a licensed driver with an automobile that is insured in accordance with
State and Agency required liability insurance.
7. Possesses and maintains current CPR certification.
SALARY RANGE: $79,040 - $85,280
This salary range is for full time - 40 hours per week.

- Establishes a plan of care which addresses all the problems identified in the assessment or demonstrates rationale for not doing so. The plan is consistent with the physician plan of treatment and signed by the physician.

- Assesses data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
