Weekend Triage Nurse


This is a full time position, which can be performed primarily from home, triaging all patient and family calls to the agency beginning Friday at 4 pm through Monday at 8 am. You will:
  • Meet with team leaders on Friday afternoon to understand all patients scheduled to be seen over the weekend
  • Coordinate all weekend care, included patients scheduled to be seen, patients with emergent conditions and new patient referrals
  • Respond to all patient and family calls, assess the patient’s condition and problem solve as necessary, including contacting physicians and assigning an agency nurse to make a visit.
  • Accept referrals for new patients and assign a nurse for admission as necessary
  • Provide clinical support for nurses in the field as needed
  • Provide accurate Monday turnover, summarizing all patients seen, and identifying patients that require immediate follow-up
Vermont RN license required; BSN preferred. Minimum five years nursing experience required; one to two years of telephone triage experience preferred. Excellent communication skills and the ability to work as part of a team are crucial.  Familiarity with the electronic health record required. This is a full time position with excellent benefits.
